2. Schedule an appointment with your MD. Your physical must be signed by an
MD or DO. Print the link below to print the physical form, and take it to your MD. Form located below.
3. Go to your local Wallgreens and purchase 2 passport photos
4. Make a copy of your birth certificate.
5. Once you have completed all five steps, and have all your paper work complete, call or text Norma Nunez, our New Registration Chair. She will issue you your Boxing Passbook/License.
